You must unlock Branching Pathways node in Nightmares Dungeon War Plan. When the ideal affix finally appears, enter that portal and clear almost the entire third floor - but be careful not to kill the floor's final boss. If you kill the boss, the entire exploit breaks.

Your primary gains come from the chests. This floor typically contains 7 to 8 chests, and each chest yields around 250,000 fragments of the corresponding gem type (using Ruby fragments as an example).
