Occult gems are special in the game. They cannot be dropped by killing enemies like Diablo 4 Gold. Instead, you need to collect specific materials and craft to obtain them. Complete Diablo 4’s base campaign and Vessel of Hatred expansion, and you’ll be able to start accessing the season’s questline in Season 7, which unlocks the crafting of Occult gems by talking to Gelena at Tree of Whispers.
The materials currently used to craft Occult gems include Fugitive Heads, Restless Rot, and various gem fragments. You can get the first two materials by completing the new seasonal event Headhunts, and the gem fragments will always accompany you throughout the game, so you can collect enough by playing the game.
