Diablo 4 Season 13 & Lord of Hatred New Uniques Revealed | The Insane builds that will completely define Meta

Apr 25, 2026 Author: D4gold

Diablo 4 Season 13 and Lord of Hatred expansion officially unlock on the evening of April 27, bringing a host of brand-new and reworked Unique items that will undoubtedly give rise to some exciting new builds.

On top of that, Unique items and Mythic Uniques are receiving sweeping changes to their affixes, and they will also interact with the new Charm system - though only players who own the expansion can take advantage of this. Now, let's dive into the new Unique items.

Diablo 4 Season 13 & Lord of Hatred New Uniques Revealed | The Insane builds that will completely define Meta

Unique Item Changes

Affix Randomization

First, fixed affixes on Unique items are becoming a thing of the past. Starting in Season 13 and onward, they will be randomly generated, which means a Unique item can roll builds that are completely useless for your character. You may have to sift through a pile of identical Unique items to find the one you actually need.

Fortunately, Horadric Cube can address this drawback. One of its recipes allows you to place three Unique items or Charms of the same name into Cube and transform them into a brand-new version of that item, saving you a great deal of time. Even more importantly, Unique items can now be Tempered, letting you manually add missing affixes.

Turning Unique Equipment into Charms

The Charm system in Lord of Hatred includes a Unique Charm slot. You can convert a Unique item into a Unique Charm to benefit from its unique effect (attributes and affixes are not inherited), freeing up a crucial equipment slot so you can equip other powerful Diablo 4 items.

Necromancer

New Unique Items:

  • Blood Wake
  • Deathgrip
  • Gravewalker's Hand
  • Hangman's Hand
  • Mace of King Leoric
  • Omen of Pain
  • Pact of Bone
  • Red Blessing
  • The Gloom Ward
  • The Undercrown
  • Vengeful Sinew
  • Will of Rathma

Reworked Unique Items:

  • Cruor's Embrace
  • Ebonpiercer
  • Gospel of the Devotee
  • Ring of the Sacrilegious Soul
  • The Hand of Naz

The Undercrown

This new helm increases the maximum number of your Skeletal Warriors and Skeletal Mages. Combined with relevant Aspects, you can summon an unprecedentedly massive army.

In addition, Season of Reckoning introduces Command skills. When you order your Skeletal Warriors to focus an attack, your Skeletal Mages will also lock onto the same target for 5 seconds. Paired with Ring of Mendeln, your undead army can unleash devastating damage.

Red Blessing

Red Blessing's effect: while you are Healthy, you gain 4 stacks of Maximum Overpower. Blood Orbs grant you 2 stacks of Overpower, and each stack of Overpower increases your damage by 10%. This amulet alone can consistently provide a 40% damage increase.

If you further combine it with Tempering affixes, Unique items, or Legendary Aspects that raise your Overpower cap, you can accumulate massive damage bonuses by collecting Blood Orbs and then detonate them all at once, nuking your enemies. Of course, you don't have to occupy your amulet slot with this effect - you can entirely convert it into a Charm.

Druid

New Unique Items:

  • Accord of the Wilds
  • Autumnal Crown
  • Dirge of Airidah
  • Fractured Runestone
  • Fury of the Wilds
  • Greenwalker's Oath
  • Greenwalker's Signet
  • Heart of Azgar
  • Ifeh's Dire Totem
  • Mark of the Old Wolf
  • Might of the Ursine
  • Purified Lightbringer
  • Thundergod's Blessing
  • Will of Stone

Reworked Unique Items:

  • Airidah's Inexorable Will
  • Dolmen Stone
  • Gathlen's Birthright
  • Greatstaff of the Crone
  • Hunter's Zenith
  • Insatiable Fury
  • Khamsin Steppewalkers
  • Mad Wolf's Glee
  • Malefic Crescent
  • Stone of Vehemen
  • The Basilisk
  • Waxing Gibbous
  • Wildheart Hunger

Greenwalker's Signet

This ring's effect: when you cast or channel a Human skill, you have a 15% chance to trigger another equipped non-ultimate Human skill, and your Human skills deal 50% increased damage.

Druids have a wealth of powerful Human-form skills, and all Companion skills count as well. While a 15% trigger chance may appear low, the number of procs becomes quite substantial when you are casting skills at a high frequency, and the 50% multiplicative damage increase is always active.

Fury of the Wilds

Fury of the Wilds' effect: when you shapeshift into Werewolf or Werebear form, you gain Berserking for 8 seconds. Additionally, while Berserking is active, you gain a stack of Ferocity every second. Berserking's damage bonus is increased by 10%.

Barbarian

New Unique Items:

  • Dark Stalker's Medallion
  • Emblem of Staalbreak
  • Might of Qual-kehk
  • Nomad's Longing Heart
  • The Open Eye of Gorgorra
  • The Relentless Heart

Reworked Unique Items:

  • 100,000 Steps
  • Arreat's Bearing
  • Bane of Ahjad-Den
  • Fields of Crimson
  • Gohr's Devastating Grips
  • Hellhammer
  • Overkill
  • Twin Strikes
  • Ugly Bastard Helm
  • Unbroken Chain

Bane of Ahjad-Den

This Unique glove has been completely reworked. Its old effect caused your next Mighty Throw to periodically detonate fire damage. Now, its new effect: while channeling Whirlwind, you automatically throw Mighty Throw periodically, and these throws deal 90% increased damage.

You get to enjoy all Whirlwind-related bonuses and Aspects while simultaneously triggering every Mighty Throw Aspectand damage modifier; higher attack speed means more frequent throws. This makes Whirlwind Barbarian considerably stronger.

The Open Eye of Gorgorra

This new amulet's effect: when dealing damage over time, you have a chance equal to your Critical Strike Chance to increase that instance of damage by 135%. Combined with Barbarian's skill tree rework, it may bring Bleed Barbarian back into the meta.

Rogue

New Unique Items:

  • Cassia's Grace
  • Desperate March
  • Etna's Lost Dagger
  • Fist of the Iron Rose
  • Gladiator's Triumph
  • Misericorde
  • Sea Lord's Fine Gloves
  • Shrouded Gift
  • The Maestro

Reworked Unique Items:

  • Bands of Ichorous Rose
  • Beastfall Boots
  • Cowl of the Nameless
  • Saboteur's Signet
  • Scoundrel's Kiss
  • Scoundrel's Leathers
  • Skyhunter

Etna's Lost Dagger

Etna's Lost Dagger's effect: when you deal damage based on two different non-physical damage types, your Imbuement skills and non-physical damage effectiveness are increased by 60% for 8 seconds. Additionally, Basic skills can now be Imbued. This may allow Heartseeker Rogue to rise again in Season of Reckoning.

Shrouded Gift

Shrouded Gift's effect: whenever you gain Stealth, you also gain Concealment. While Stealthed and for 8 seconds after breaking Stealth, you gain 30% damage reduction.

Assuming you can enter Stealth frequently through some means, Concealment effect will be triggered repeatedly, potentially providing Energy, extra critical strikes, dodge chance, or even guaranteed critical strikes and Vulnerable.

Spiritborn

New Unique Items:

  • Echo of Kwatli
  • Protean Heart
  • Thundergod's Blessing
  • Widow's Web

Reworked Unique Items:

  • Path of the Emissary
  • Scorn of the Earth

Echo of Kwatli

Echo of Kwatli's unique effect fully replicates Spiritborn key passive Vital Strikes: it increases your damage against Vulnerable enemies by 100%. After making an enemy Vulnerable, your next direct damage restores 5% of your Life and 10 Vigor, and removes Vulnerable effect.

Protean Heart

Protean Heart Amulet's effect is similar to Adaptive Stances key passive. The base Spirit of the skill you cast provides different bonuses. Casting a skill with a different base Spirit swaps the bonus and increases your Core Stats by 100% for 7 seconds.

Sorcerer

New Unique Items:

  • Drognan's Anguish
  • Emberfury
  • Fang of the Vipermagi
  • Gift of Frost
  • Levin Grasp
  • Molten Band
  • Onyx Soul
  • Raiment of the Sea
  • Rimeblood
  • Shanar's Resonance
  • Staff of Zerae
  • Thundergod's Blessing
  • Vision of the Firestorm

Reworked Unique Items:

  • Axial Conduit
  • Blue Rose
  • Esadora's Overflowing Cameo
  • Flamescar
  • Fractured Winterglass
  • Galvanic Azurite
  • Hail of Verglas
  • Iceheart Brais
  • Ophidian Iris
  • Sidhe Bindings
  • Starfall Coronet
  • Strike of Stormhorn
  • Vox Omnium

Drognan's Anguish

Drognan's Anguish's effect: when you cast a Burns skill, you burn 5% of your Maximum Life over 10 seconds. Meanwhile, based on the proportion of Life you are burning, your Burning damage is increased by up to 220%. This item likely requires you to stack Life to achieve the highest possible damage bonus.

Onyx Soul

Onyx Soul is a Unique Focus with the effect: Frozen Orb now returns to your character and deals 100% increased damage.

Starfall Coronet

Starfall Coronet has been completely reworked: casting Meteor consumes Overpower stacks. For every two Overpower stacks consumed, an additional Meteor falls. Meteor damage is increased by 70%.

Paladin

As for Paladin, while it is also part of Lord of Hatred expansion, it was already released in Season 12. In Season 13, it mainly receives balance adjustments, with very few changes to its Unique items.

Warlock

New Unique Items:

  • Ae'grom's Schism
  • Cage of Madness
  • Cowl of Malefic Torment
  • Eye of Baal
  • Gauntlets of Sheol
  • Hellhound's Sabatons
  • Kabraxis' Wall
  • Seal of the Ophanim
  • Seed of Horazon
  • Spine of Tathamet
  • The Fecund Seal

Warlock is a completely new class of  Lord of Hatred, so all of its Unique items are brand new and packed with exciting new mechanics.

Ae'grom's Schism

This Unique item boosts minion damage by 50% and summons the other half of Ae'grom. It is essentially a must-have for summoner Warlocks.

Cage of Madness

This item replaces your Evade with an angry Lunatic that deals Command Fallen's damage. It lasts for 15 seconds and its damage increases by 100% every second, meaning its damage can stack to an absurd degree. This could lead to a very interesting build.

All Classes Equipment

New Unique Items:

  • El'druin, Sword of Justice (Mythic Unique)
  • Signet of Pelghain

Reworked Unique Items:

  • Banished Lord's Talisman
  • Frostburn
  • Shroud of False Death (Mythic Unique)
  • Locran's Talisman

Banished Lord's Talisman

Its new effect: after consuming 275 primary resource, you gain 4 stacks of Overpower. Your Critical Strikes deal 18% increased damage per stack of Overpower. It can be combined with items like Starfall Coronet that require Overpower stacks.

Locran's Talisman

This amulet's new effect: Critical Strike damage is increased by up to 150%, but your Critical Strike Chance is reduced by 50%. You can push your Critical Strike Chance well beyond 100% through other means to offset the 50% penalty and gain an immense critical damage boost.

The above are some of the new and reworked unique gear items brought to Diablo 4 Lord of Hatred. The addition of randomized affixes may raise some concerns about the item hunt, but once you figure out how to make good use of Horadric Cube and Tempering, putting together new builds will feel completely intuitive.
