Survive the New Relentless Butcher in Diablo 4 Patch 2.6.0 PTR | Are Bloodied Rewards Worth the Risk?

Feb 04, 2026 Author: D4gold

Diablo 4 2.6.0 PTR launched on February 3rd! You have a week to experience and test the bloody feast that Season 12 will bring! Of all the new content, the most attractive and challenging is undoubtedly the more frequent appearance of The Butcher.

Previously, this bloodthirsty and dangerous boss only appeared randomly, but now, in addition to that, they can be guaranteed to appear via Bloodied Sigils, and making the surrounding enemies even more dangerous!

Based on this, we will use Infernal Hordes mode as the battlefield to test the experience of the new The Butcher encounter in 2.6.0 PTR and whether it's worth the risk of death.

Survive the New Relentless Butcher in Diablo 4 Patch 2.6.0 PTR | Are Bloodied Rewards Worth the Risk?

The Butcher combat mechanics

This boss will appear in any dungeon and at any character level, and will then hunt you down. If you hear the exclusive background music and the signature line "Fresh Meat!" during your adventure, it means you are about to face dire consequences.

The Butcher primarily damages you with his one-handed axe weapon, The Butcher's Cleaver, and a hook that pulls you in front of him. Being hit by the cleaver can cause a slow and fear effect.

Therefore, a common strategy for dealing with The Butcher is to run to areas with complex terrain, using obstacles to avoid the hook while attacking the boss until it's defeated. Generally, running clockwise around him is more likely to miss his main attacks.

During the fight, use control skills to fill the boss's stun bar, rendering him helpless for 12 seconds. If you have skills that grant invulnerability, you can better escape the stun effect.

If you encounter The Butcher at a low level, escaping as far as possible may be your only way out; otherwise, you'll be killed by the boss and disappear into a pool of blood until you encounter him again.

Encountering The Butcher in 2.6.0 PTR Infernal Hordes

The gameplay of Infernal Hordes is wave-based; once you kill enough regular enemies to unlock and defeat the final boss, you can start enjoying the rewards.

But now, if you want to add some extra excitement, you can farm Bloodied Sigils and use them to make any Diablo 4 dungeon adventure more difficult because of Relentless Butcher affix effect.

These sigils will start dropping through various gameplay mechanics after Torment 1 difficulty is unlocked, making the affected gameplay more challenging while ensuring you get Bloodied Items rewards.

Once the affix effect takes effect, these new The Butchers will appear and start hunting you. They are now healthier, harder to kill, and deal more damage. If you're unlucky enough to trigger more random The Butchers at once, escaping will be difficult.

Best Strategy to deal with The Butcher

While the mechanics of endgame events that allow Relentless Butcher affix to take effect differ, the strategies for dealing with The Butcher are universal.

First and foremost, it's crucial to understand the boss's attack patterns and weaknesses, as we've already discussed. Secondly, it's highly recommended to equip a sufficiently tanky build.

Based on the current state of the game, we prioritize Paladin players for this boss fight, as this class currently offers the best balance of damage and defense in Diablo 4. Moreover, as you know, new classes are always stronger in the short term.

Although the changes in 2.6.0 PTR have weakened Paladin skills and some Paragon nodes, some have been strengthened. Therefore, overall, this class is likely to remain dominant in Season.

Based on this, if you want to further enhance your damage output, a build centered around Arbiter of Justice form is the best choice. Combining this with rare Diablo 4 items, such as Heir of Perdition, will further enhance the build's effectiveness.

Of course, choosing other classes is perfectly feasible, but you still need to ensure your damage output and tankiness are high enough.

Additionally, you can use the new unique items added in Season 12 to craft more builds, utilizing their effects to increase damage output when encountering The Butcher, such as Blood-Mad Idol or Rustbitten Dirk.

These items can be obtained by completing endgame events or farming specific bosses, and in the process, you will also obtain more Bloodied Sigils, providing a source for unlocking more The Butcher boss fights.

Is The Butcher boss fight worth unlocking?

Based on PTR experience, this boss seems much more troublesome than when it appeared randomly in the past, especially if you also have to deal with other normal enemies that are stronger due to Bloodied Sigils effect.

Fortunately, however, the loot from defeating The Butcher is considerable. Besides basic gold and XP, there are also many Bloodied items.

This special item adds extra modifiers to corresponding regular items, enhancing them and making your character and builds using it more reliable.

Therefore, in summary, this boss fight is worth utilizing, provided you can handle it and bring the best gear and builds; otherwise, frequent deaths would waste time.

That concludes our introduction. If you want to truly understand how terrifying The Butcher is, the best way is to experience it for yourself! So why not try it out on PTR now according to our guide!
